Health concerns can come up at any time of the day or night. That’s why 24-hour access to a registered nurse is so valuable. These trained medical professionals can answer employees’ questions about everything from allergies to earaches, chronic conditions, and even advise what level of care should be considered, including emergency or urgent care.
Employees with chronic conditions like asthma, diabetes and chronic obstructive pulmonary disease (COPD) can turn to our ConditionCare nurse coaches for one-on-one support to help better manage their conditions. Members will receive the information, tools and expert guidance they need to help avoid complications and time away from the job.
Mothers-to-be receive personalized support and guidance, helping them achieve healthier pregnancies and deliveries. With their own team of obstetric specialists, expectant members get access to expert information and direction throughout their pregnancy and postpartum period.
When members are hospitalized for a major illness or injury or are struggling with multiple health issues, our registered nurse case managers work to help them get the best care possible. These trained health experts are highly skilled at assessing and supporting the whole person, not just the health issue, through convenient video chat and automated follow-up phone care.
When people don’t feel well emotionally, relationships, work and even physical health can be affected. With our Behavioral Health Care Management programs, members and their families receive strong support from our licensed mental health care managers to ensure they’re getting all the benefits available to them under their health plan.
Our teams collaborate with providers to ensure members receive the right care at the right time in the appropriate setting, aligning medical services with the member’s plan benefits. Additionally, our concurrent review service and pre- and post-certification reviews help reduce costs as well as gaps in care.
A quick, convenient way for employees to get the medical care they need when their own doctor isn't available. Using LiveHealth Online, employees don't have to take time off work or spend a day at the urgent care center when they are feeling under the weather. Now they can see a doctor 24/7 on their computer or mobile device. They can talk face-to-face with a board certified doctor to get medical advice, a diagnosis and even a prescription if needed with no waiting or appointments.
The Online Wellness Toolkit gives members the tools they need to set and achieve their unique health goals. It includes a Health Assessment to help them figure out their health risks, guidance to help them lower those risks, personalized trackers to check how they are doing and fun activities and challenges to keep members engaged.
This program encourages employees to complete a confidential health questionnaire that assesses their physical and emotional health and identifies any potential at-risk behaviors. Based on their answers, a personalized well-being plan is created, with recommendations and a wellness “roadmap” that helps guide them toward improved health.
MyHealth Advantage analyzes member health data and history, and then compares it with medical best practices, revealing possible health risks, care gaps or ways members could save money. Personalized and action-oriented communications called MyHealth Notes help connect members with their providers and direct them to make healthcare choices that minimize risk and maximize wellness.
When members experience health or medical issues after hours, we can help them find the right care for non-emergency situations. By educating employees on alternatives to the ER, such as urgent care centers, walk-in doctor’s offices and retail health clinics, they’ll enjoy shorter wait times and lower costs for care, which also helps businesses keep their costs down as well.
Our Shopper Programs promote appropriate and affordable care for certain routine imaging, sleep and surgical procedures. Following a clinical appropriateness review, the Shopper Program may provide proactive outreach to members to let them know if there is a lower cost facility available for that procedure. With this information, members can take charge of their health care costs.
We believe that when it comes to the health of your employees, the whole picture is best.
With Anthem Whole Health Connection℠, we can look at the full spectrum of your employees’ health. Using claims and history data, we can connect the dots between the types of care they get: medical, pharmacy, dental, vision, disability and behavioral health. This gives doctors and other health care providers a more holistic profile of their patients’ overall health — and leads to better care, better outcomes and lower costs.
Enhanced Personal Health Care (EPHC) is our approach to patient-centered care. We work closely with doctors to drive lower costs for employers and better outcomes for members. EPHC helps doctors do what they do best — take care of their patients and gives our members better access to the care they need at the right time. We do this by:
• Paying doctors for value over volume when they improve patient health, meet quality standards and lower costs.
• Giving doctors added support so they can spend more time with patients and coordinate their care with other doctors.
• Improving the patient experience with better access to a primary care physician (PCP) who cares for the “whole person.”
Check out the wealth of informative, interactive health and wellness tools available to help your clients communicate the benefits of these programs and services to their employees. In addition to the above programs, employers may also have the option to include our Employee Assistance Program, which aids in stress management, financial planning, child care and other family- and work-related concerns. Additionally, some plans include incentives to give members a little extra motivation and increase participation in health management programs.
